Relaxation Doesn't Cure Anxiety. Why Do We Keep Prescribing It?
You cannot cool a boiling pot by dropping in ice cubes. You have to turn off the stove. Here is the mechanism, and exactly how to run it.

She was very good at relaxing.
Give her the right conditions and she could take herself all the way down. The cushion, the quiet morning, the guided track, the breath count. Ten years of practice. On a calm Thursday with the house empty, she could drop her heart rate like she was setting down a bag.
Then something would set her off.
An email. A tone of voice. Sometimes nothing she could name at all… her body just decided, off some cue she never consciously registered, that this was the moment. And it popped. Chest, jaw, throat, that electric floor-dropping feeling. Every single thing she knew how to do stopped working. The breath count did nothing. The track did nothing. She would sit there executing the technique correctly and feel the alarm keep screaming right through it.
She came to me thinking she needed better relaxation tools. She had plenty. That was never her problem.
Here is what I told her, and it is the thing I got wrong myself for years, back when I taught people to calm down as if calm were the whole job.
Relaxation forces the nervous system into calm. Regulation trains the nervous system to return to calm on its own. Those are two different skills, and she had spent a decade practicing the first one under ideal conditions while never once training the second.
So the moment conditions stopped being ideal, she had nothing.
You don’t force calm. You train calm. This whole article is how.
Ice cubes in a boiling pot
Picture a pot of water at a hard boil.
You want it to stop boiling, so you start dropping in ice cubes. And it works, a little. The bubbling settles for a second. So you drop in more. Settles again. You get quite skilled at this. You learn to time the cubes. You buy nicer ice. You build a whole morning routine around the cubes.
The stove is still on.
That is relaxation applied to a nervous system that is running hot. Every calming technique you have ever been handed is an ice cube. Box breathing, the cold plunge, the sound bath, the app. They are real, they do something, and the second you stop, the water goes right back to a boil, because nothing you did ever touched the burner.
The story underneath is the burner. The belief that says I am not safe unless I am perfect, I have to perform or I am done, I do not get to take up space. That story generates heat continuously, in the background, whether you are meditating or not.
Turning off the stove is a different job than cooling the water.
And here is the part that sounds backwards. To reach the burner, you have to let the pot boil on purpose.
What the zebra does
A zebra is grazing on the plains. The lion comes out of the grass.
Everything fires at once. Heart rate triples, blood dumps into the legs, vision narrows, and the zebra runs for its life. Full sympathetic burst, everything the body has.
It escapes. And then, in the minutes after, it stands there and shakes. Legs, flanks, the whole animal trembling. It blows out a few enormous breaths. And then it goes back to eating grass.
Back to grazing. Same field. Same lion still out there somewhere.
Robert Sapolsky built a whole book around why that zebra does not get the stress diseases we get, and the answer was never that the zebra is calmer than you. That zebra was, for ninety seconds, more activated than you have been in a decade. The difference is that it went all the way up, then all the way back down, and the whole thing was over.
The energy got used. The cycle closed. The system reset.
Peter Levine built an entire therapy on that shaking, the idea that the tremor is how the animal discharges what it mobilized. I find it compelling and I use versions of it clinically. I will also say plainly that the evidence for that specific claim is thinner than its popularity suggests. Take the shaking as an observation worth sitting with, not as settled physiology.
What is not in dispute is the shape. Up hard, down fully, done.
Now look at what we do instead. The surge comes and we immediately try to shut it down. We breathe it away. We talk ourselves out of it. We never run, we never shake, we never finish. The charge stays in the system and we go back to our desk.
Do that for twenty years and you teach your body one thing very thoroughly: activation is dangerous and must be escaped. So the body gets more sensitive to it, not less. That is not regulation. That is avoidance training with a wellness label on it.
You are not a zebra. You shed differently.
Here is where people get confused when they try this, so let me save you the confusion.
You are probably not going to stand in your kitchen and tremor like an animal on the savanna. Some people do. Most do not. We are built differently, and the human version of discharge tends to come out through the emotions rather than through the legs.
So you need to know what completion actually looks like in a person. There are three signs, and when one shows up, that is the rep landing.
You cry. Often out of nowhere, often about something that seems unrelated, often with no story attached to it at all. This is the most common one and honestly the best one.
You shake. Hands, jaw, legs, a full-body shiver that has nothing to do with being cold.
You get some other kind of energetic release. A wave of heat. A sudden enormous breath you did not decide to take. Chills. Sometimes laughter that comes up as hard and unbidden as the crying would have.
None of those are the thing going wrong. Every one of them is the thing going right. The charge that has been sitting in your system with nowhere to go is finally going somewhere.
And I want to say something specific to the men reading this, because I have watched hundreds of you get right up to the edge of this and slam the door.
Let yourself cry.
Not in front of anyone if you do not want to. Not as some performance of vulnerability. Just stop clamping it when it rises. You have spent your whole life learning to hold the line while the pressure is on, and that instinct is not wrong, it is genuinely useful, it has probably carried you and the people who depend on you through some real things.
But hold this next to it: warriors don’t cry during the battle. They definitely cry after.
The holding is for the battle. This is after. This is the part where you set it down, and if you never set it down, you just carry the whole war around inside a body that is still braced for it.
There is a number for this
Cardiologists have a measurement called heart rate recovery. Push someone to maximum on a treadmill, stop them, and count how far the heart rate falls in the first sixty seconds. That number is one of the cleanest windows we have into vagal function, because that first-minute drop is your parasympathetic system switching back on.
It is also one of the more sobering numbers in medicine. In a study of more than two thousand adults published in the New England Journal of Medicine, a sluggish first-minute recovery predicted death from any cause over the following six years, independent of fitness and independent of what the heart looked like on the test.
Read that again. Not how high you can go. How fast you come back down.
And here is the part that matters for you: it moves. Trained athletes have dramatically faster recovery than untrained people. People in heart failure have blunted recovery. It improves with training and it degrades without it.
That is regulation, measured. Not a vibe, not a mood, not how peaceful you felt during your meditation. A trainable rate of return.
You do not train it by staying calm. There is no way to practice coming back down without first going up.
They tested this against relaxation, and relaxation lost
Years ago I would have told you all of the above was mechanistic reasoning plus what I have watched happen with the thousands of clients I have sat with. Then somebody ran the experiment.
Published in Frontiers in Psychiatry February 2026. Seventy-two adults with diagnosed panic disorder, all off medication, randomized into two groups for twelve weeks.
Group one did brief intense exercise. Supervised walking with repeated thirty-second all-out sprints thrown in, building from one sprint per session up to six. Deliberately spiking the exact sensations a panic patient is terrified of… pounding heart, hard breathing, that flooded feeling.
Group two did Jacobson’s progressive muscle relaxation. The gold-standard calm-down protocol. Three times a week, forty-five minutes.
The sprint group did substantially better. Lower panic scores, fewer attacks, and the gap was still there at twenty-four weeks, three months after the program ended.
Sit with that design for a second. Relaxation training was not a straw man here. It was the control arm, the respectable comparison, the thing we have been handing anxious people for fifty years. And deliberately spiking people’s nervous systems, safely and repeatedly, beat it.
This is not new in principle. Broman-Fulks and colleagues showed twenty years ago that high-intensity aerobic exercise reduces anxiety sensitivity, which is the fear of your own bodily sensations, and the leading explanation was always that hard exercise IS interoceptive exposure. It floods you with the sensations you are afraid of while nothing bad happens. The nervous system updates its model.
Which reframes the entire anxiety conversation. Some people can breathe themselves down and some people cannot, and we have spent all our energy arguing about which technique suits which group. Neither group is being trained. They are both being handed ice cubes.
Four ways to run the wave
The training unit is a wave. Up hard, then down deliberately. That is one rep.
You have four accelerators and four brakes, and they all drive the same machinery. Pick the pair you will actually do.
Movement. Up: sprint, hill, stairs, burpees, heavy carries. Hard enough that talking is not really an option. Down: slow walking, and then slow stretching, foam rolling, lying on the floor with your legs up the wall. Anything unhurried and physical. Keep it going a few minutes longer than you think you need to, because the coming down is the part you are actually training.
Breath. Up: fast forceful double inhales through the nose, belly then chest, with the exhale falling out on its own, no control on the way out. Down: long slow exhales with a hum, letting the vibration sit in your throat and chest.
Music. Up: something driving, loud, aggressive, whatever actually spikes you. Down: slow, ambient, spacious. This one is the easiest to underestimate and the easiest to add. Music will drag your physiology along behind it whether you consent or not, and it stacks on top of any of the others.
Touch and percussion. Up: jump, tap hard on your chest and thighs, pound your feet into the floor, shake your arms out. Down: slow stroking. Cross your arms and stroke gently down from shoulder to elbow, over and over, slower than feels natural.
Any pair works. Stacking two or three works better. My own default is movement plus music, then hum plus stroking on the way down, and the whole thing takes under ten minutes.
But here is the thing almost everybody gets wrong, and it is the reason people do breathwork for a year and stay exactly the same.
The wave by itself is just cardio.
The wave is only half of it
Running the wave trains the return. It builds the capacity. It does not, on its own, turn off the burner.
For that, two things have to happen while you are up there.
One: you have to bring the charged material with you. The anxiety, the memory, the belief, the thing that has been running you. It has to be in the room, at full volume, while the system is hot.
Two: you have to hold it differently than you normally do. Not fight it. Not become it. Watch it.
Why those two, specifically? Because of what showed up in the research this year.
A team at RIKEN in Japan led by Joshua Johansen, published in Neuron, traced what happens inside the amygdala at the exact moment a fear memory is recalled. Noradrenaline (the brain’s own adrenaline) pours in from a small brainstem cluster called the locus coeruleus. It lands on beta-adrenergic receptors on one specific population of amygdala neurons. Then a protein called CRTC1 physically travels from the synapse into the nucleus of the cell, from the connection point down to the DNA, and switches on the genes that let that memory be reformatted and re-saved.
No noradrenaline, no rewrite.
The alarm chemical is the pen.
We have had the mirror image in humans for years. Alain Brunet’s group had trauma survivors recall their trauma after taking propranolol, which blocks those same receptors. Recall it with the pen blocked and the memory gets re-saved carrying less charge.
So the chemical your body makes when you are activated, the one an entire industry is teaching you to suppress, is the same chemical that makes your story editable.
And your breath is wired straight into the supply. A Stanford team found a small population of neurons in the preBötzinger complex, the brainstem pacemaker that generates your breathing rhythm, projecting directly to the locus coeruleus. One synapse from the breath generator to the noradrenaline factory.
That is not a metaphor about breath and mood. That is a wire.
Your stories were not installed in calm states. They were installed in charged ones… the moment of humiliation, the moment nobody came, the moment you decided what you were. High arousal is what seared them in. Sedation cannot reach them. You can soothe a story all day long and never once open it.
So you go back in with the heat on. But you go in differently.
Meet the rude guest
This is the part that makes the whole thing work, and it is the part nobody teaches.
When the charge comes up, you have always done one of two things. You fight it, or you become it. You brace against the anxiety and try to make it leave, or you disappear into it until there is no difference between you and the feeling.
There is a third option, and it changes everything.
Treat it like a person who just walked into the room.
Somebody has come in uninvited and is behaving badly. Loud, erratic, rude, waving their arms, insisting the building is on fire. That is your anxiety. That is the old story. It has burst into your afternoon and it is making a scene.
You have two good ways to handle a guest like that, and both of them work.
The first is to simply not react. Look at them. Do not argue, do not leave the room, do not join in. Just watch. And here is what happens when you actually do it: the performance starts to look a little ridiculous. The flailing needs an audience to stay convincing, and you stopped being one. There is often a moment where you almost want to laugh, and that moment is worth more than an hour of trying to calm down.
The second is to turn toward them and engage. Okay. You are here. I am listening. What do you have to teach me?
Either way, notice what just happened. You are no longer inside the feeling. You are across the room from it. That gap is the entire technology, and everything else is just how to make the gap wide enough to stand in.
Here is how you make it wide enough. Three moves, in order, while the system is hot.
Name it, and welcome it. Out loud if you can. Hello anxiety. I see you are back again.
The welcome is not a nicety. Greeting something is a fundamentally different act than bracing against it, and your body knows the difference immediately. Naming it also does something specific: the second you say the word, you have made it a thing you have rather than a thing you are.
Locate it. I see you like to hang out in my solar plexus.
Find where it actually lives. Throat, chest, gut, jaw, the backs of the arms. Feelings that have an address stop being everywhere.
Watch its personality. This is the one that sounds strangest and works best.
How is it dressed? What color is it wearing? What is the texture of the clothes, rough or slick or heavy? Is it fast or slow? Hot or cold? Is it shouting or sulking? Does it look like anyone you know?
Then just watch. Detached, curious, the way you would watch a strange bird in the yard. Not fixing, not analyzing, not making it leave.
I know how this reads on a page. Do it one time, while your heart rate is up, and you will feel the floor shift. The dread that felt infinite turns out to have edges, a color, a location, and a personality. And a thing with edges can be held, examined, and set down.
Why the guest changes the wiring
Here is the mechanism underneath the strange little game.
Your old story runs on a prediction. It predicts that when this feeling arrives, you will do what you have always done. Fight it, run from it, numb it, or drown in it. Every single time you do one of those, the prediction comes true and the story gets re-saved exactly as it was. That is why twenty years of coping never changed anything. You were confirming it, over and over, with excellent technique.
So you break the prediction. The feeling shows up expecting a fight, and finds somebody calmly asking what color it is wearing.
That mismatch has a name in the research. Prediction error. It is the signal that tells the brain this file is out of date and needs updating, and without it, recall just re-saves the same story. Merel Kindt’s lab in Amsterdam has been the most rigorous on this point: reactivation alone is not enough. Something has to violate the expectation.
The activation opens the file. The mismatch is what lets you write in it.
That is the whole mechanism, and it is why neither half works alone. Breathwork without the story is cardio. Talking about the story while calm is a book report. You need both, in the same window.
Too hot cements. Too cold does nothing.
The dose is the whole game, and getting it wrong is how people either get hurt or get nothing.
The relationship is not a straight line. It is a curve. Karim Nader’s lab at McGill showed that under very high noradrenaline, true flooding, panic-level activation, that same locus coeruleus input flips its job. Instead of opening the memory it slams it shut and cements it harder. When they blocked the excess signal, memories that had been untouchable became editable again.
In my clinical practice we call it the heat rule. You want the emotion around a 6 to 8 out of 10.
Cold recall, describing the memory like a news report, never opens the file. That is most talk therapy. Flooded recall, a 9 or a 10, welds it shut harder. That is why raw exposure retraumatizes some people.
Warm, not burning. Activated, not drowning. If you cannot watch the guest anymore because you have become the guest, you have gone too far. Back it down.
One flag and then I will stop flagging. The Neuron work was done in rats. The heart rate recovery data and the panic trial are human. The full chain, this wave opening this window in this order in a person, has not been run as a single experiment. That connection is mine, built over twenty-plus years of clinical work, and parts of it may be wrong.
The whole thing, start to finish
Ten minutes. No equipment.
1. Pick your pair. Movement, breath, music, percussion, or a stack of them. Whatever you will actually do.
2. Go up. One to three minutes. Not twenty. You are aiming for a 6 to 8 out of 10, not a 10. Short is not a compromise. Short is the dose.
3. Bring the guest in. While you are hot, let the thing come. The anxiety, the memory, the belief that says I am not safe, I am too much, I will be left.
4. Name it, locate it, watch its personality. Hello anxiety, I see you are back. I see you like my solar plexus. What are you wearing today? Watch it with interest and no reaction. Or engage it and ask what it came to teach you.
5. Finish the wave. Let the energy complete. Move, shake, push, let the breath go ragged. Do not cut it off early. And if the tears come, or the shaking, or the wave of heat, let all of it happen. That is not the wheels coming off. That is the discharge, and it is the whole point.
6. Come down on purpose, into a new feeling. Long slow exhales with a hum. Cross your arms and stroke slowly down them. Slow stretching, the floor, the slow music, a walk. And while you descend, deliberately generate gratitude, appreciation, warmth toward the younger version of you who came up with that story in the first place. Then let it go soft. Bliss if you can get there. Woosah.
7. Give it the meaning. This is the last beat and it is not optional. Say it plainly to yourself: this was not a frustration, it was an initiation. That was a rep. I am training my nervous system to come back.
You are not enduring the hard part. You are using it. The thing that used to happen to you is now the thing you train with.
Then, within the next few hours while the material is still soft, do one small thing the old story says you cannot do. Send the message. Ask for the thing. Say no. That is what seals it.
One caution, said plainly because I am a clinician and I would say it to you in the room. Fast forceful breathing can make you lightheaded or make you faint, so do it sitting or lying down, never in or near water, and never while driving. If you have a heart condition, are pregnant, or carry a panic or trauma history that runs deep, do the intense piece with your doctor’s blessing and the emotional piece with a skilled human beside you. Every session of that panic trial was supervised. That detail matters.

You don’t force calm. You train calm.
She started running waves three times a week. Three minutes hard, the guest invited in and looked at, the hum, then the sitting still.
The first few times, she cried. Not about anything she could point to. She told me she felt embarrassed about it and then a week later told me it was the best she had felt in years.
What changed first was not her baseline. It was her recovery. The thing that used to own her whole afternoon started giving the afternoon back. Then the evening came back. Then, eventually, the trigger stopped being much of a trigger, because a story that has been opened while hot and met with curiosity instead of a fight does not keep firing the same alarm.
She is not calmer than she was ten years ago. On a good quiet Thursday she could always get calm. What she has now is the thing she actually needed, which is a nervous system that comes back.
Most people are not being chased by a lion. They are being chased by a story, and that story sits under the pot running the burner all day, every day, while they stand over it dropping in ice.
You were never bad at calming down. You were just cooling the water.
Go turn off the stove.

This is one of the most important distinctions in the anxiety conversation.
Relaxation is not the opposite of anxiety. It is a temporary override, a way to suppress the signal without resolving its source.
Anxiety is not a lack of relaxation. It is a decoherence signal. It is the nervous system telling you that the biofield is unstable, that the EZ water is de-cohered, that the organism is not receiving the coherent signal it needs to orient toward safety.
Relaxation can mask the symptom. But it cannot restore the field.
What actually resolves anxiety is not relaxation, it is coherence restoration. It is the slow, steady process of re-entraining the nervous system to a signal that the body trusts.
I have seen this in my own body and in clients: when the field reorganizes, the anxiety simply loses its footing. It does not need to be suppressed, it dissolves.

From a Sufi perspective, the article’s central image of turning off the stove rather than continually cooling the water is very meaningful.
Relaxation may quiet the surface, but the deeper work is to discover what keeps the inner fire burning.
In Sufi language, this can be understood as turning toward the patterns, fears, and attachments that keep the nafs unsettled rather than simply trying to suppress their symptoms.
The article’s invitation to observe anxiety without becoming it also resonates with the Sufi practice of witnessing: noticing what arises within without immediately identifying with it.
There is a quiet freedom in being able to say, “This is arising in me, but this is not all of who I am.”
What I especially appreciate is the article’s emphasis on returning.
The goal is not to remain permanently calm, because life will inevitably bring difficulty, fear, grief, and uncertainty.
What matters is learning to return—to presence, awareness, and ultimately to the deeper ground within.
From a Sufi perspective, however, I would take this one step further: the “burner” is not only psychological conditioning; beneath all our stories is the restless tendency of the self to seek security in what is constantly changing.
The deeper turning is therefore not merely toward a regulated nervous system, but toward the Real, where the heart can gradually learn that it does not have to be ruled by every wave that passes through it.
